MINUTES
EASTHAM  BOARD OF HEALTH MEETING
THURSDAY , OCTOBER 18, 2007
AT 3:P.M.


PRESENT:        PAUL LOTHROP, CHAIRMAN,  DR. STAN HOLT,
                        PAT LARIVIERE,   DR. JAMES TAYLOR,
                        JANE  CROWLEY,  EASTHAM HEALTH AGENT
                        (BEVERLY  KNOX ABSENT)


OPENING STATEMENTS:
                        
Jane Crowley informed the Board that 2 Flu Clinics would be held at the Senior Center on Thursday November 29 and December 6 from - 9 to 12 . Residents should call the Center to make an appointment.
The Health Agent also informed the Board that Novenber 5-9 the rabies bait  project would begin. Public notice will be put on the Town Web Site as well as in the newspaper.


REVIEW



1.      Gorge Reilly (Land Design Associates) presented the plan with requests for provisional approval Title 5 System with Orenco Advantex Technology. Finding that the applicant has submitted plans with secondary treatment the Health Agent recommended approval with the following conditions:

1.      Routine water and Voc quality test results be provided of existing well location to assure potability
2.      No increase in design flow be allowed

3.      Secondary treatment and monitoring under provisional use approval conditions per DEP regulations including testing quarterly for total nitrogen, BOD, TSS, Nitrate (or 2 times per year for seasonal use).  Effluent nitrate-nitrogen not to exceed19mg/l. Operator to report results via Carmody Data base provided through Barnstable County Health or directly to the Eastham Health Department if not available. Operation and maintenance agreement to be submitted to Eastham Board of Health.  Operation and maintenance agreement to be required for life of the system.


Deed restriction for 3 bedroom use and use Orenco Advantex alternative technology requirement to be recorded on the deed  
5.      Designer to verify proper operation of the system prior to issuance of Certificate of Compliance
6.      Any increase in habitable square footage would require Board of Health review


Designer to add vericom monitoring system
Pump chamber should include steel covers and D box and SAS covers within 6"final grade
Verification of medium sand beneath area where leach area is at the time of installation.
Dr. Stan Holt made the motion to approve the plan with the 9 conditions.
Pat Lariviere seconded the motion.

Judy Bersin (Ryder/Wilcox)brought in revised plans with secondary treatment using Singulair 960-600 NR. The Health Agent recommended approval with the following with the following conditions

 Grandoni & Pinchook     710B Campground        Map-7A    Parcel-32

Mr. William Pinchook  request the  Eastham Board of Health reviewed the project to install a new foundation under the dwelling at 710 B Campground Road.  This property has a capacity for 4 bedrooms (2 bedroom in each dwelling) with a total flow of 440gpd.  The property also has a Title 5 system with Advantex treatment installed 6/13/2007.  Since this is located on an environmentally sensitive lot 5,784 sq. ft. with 10 existing variances the building project was subject to Board of Health review.
Dr. James Taylor made the motion to approve this request.
Dr. Stan Holt seconded the motion .

Breakwater Holdings     4665 State Highway       Map-8   Parcel-290
       (Paul Lothrop recuse himself from this hearing)

Timothy Klink and Attorney Ben Zehender came before the Board to answer
           questions regarding violations on the property at 4665 State Highway which

          need to be corrected..The Board approved the request  submitted on  October 16, 2007 stating Mr. Klink would  bring unit #4 of 4665 State Highway into compliance.  In order to accomplish this the following construction work must be done:

1.      Removal of all kitchen cabinets and countertops
2.      Removal of all appliances                               


3       Removal of all plumbing fixtures in the kitchen
4       Disconnect all plumbing lines as required and inspected by plumbing inspector
5.      Obtain necessary permits for the demolition and call for the necessary inspections upon completion of the work.


Submit letters from both the plumbing inspector and electrical inspector   verifying the status of the property
7.      The BOH requires confirmation that work meets all appropriate electrical and plumbing code standards and all necessary permits and inspections have been obtain as required.
Dr. Stan Holt made the motion to approve the  request submitted Oct. 16, 2007 with the 7 conditions.
Dr. James Taylor seconded the motion
 THE  VOTE WAS 3-0 FOR APPROVAL




David Hale               75 Bayview Drive               Map-13     Parcel-13    

Judy Bersin (Ryder/Wilcox) submitted revised plans to change the secondary
          technology from Advantex AX20 to Singulair 960-600 NR.
Dr. Stan Holt made the motion to approve the change in technology.
Pat Lariviere seconded the motion.
SO VOTED UNANIMOUSLY
